Pre-departure - Medical

All passengers must undergo an medical examination no more than one year prior to the date of final disembarkation. A licensed physician must complete all sections of The Scholar Ship Medical Forms A and B.

Form A must be completed by the passenger and reviewed and verified by a licensed physician.

Form B must be completed by a licensed physician to confirm the passenger is physically and emotionally fit for The Scholar Ship Program.

Both Forms A and B must be submitted to The Scholar Ship program no later than sixty (60) days prior to the date of embarkation.

Maternity Notification form (Form C) is also a requirement for participation if a female participant is pregnant.

The information contained in the required forms will be used if necessary solely as an aid to provide health care while you are a passenger on The Scholar Ship and to determine fitness for participation. It is strictly for the use of The Scholar Ship staff, and the staff of The Scholar Ship’s parent or affiliates and their respective agents and consultants and will not be released to others without your prior knowledge or consent unless required by law.
